The messages will be saved in the Prestashop's SAV (Customer Thread) and you will be notified by email at every subscriptions.

Características

With this module, you will be able to create a form that reflects exactly what you want. You will be able to create as many fields as you want, with any types of fields, including :

Text
Select
Checkboxes
Radio buttons
Textarea
Product attributes list

Moreover, you'll be able to define in which categories to show your form. This will let you place the form in specific parts of your catalogue instead of the whole website.

Absolutely everything is editable in the form :

The title
Any message before the fields (you can leave it empty)
All the fields are editable (with the possibility to add, modify or delete them).
The "thank you" message, displayed once the form has been submitted.

You'll also be able to show WHERE to show the form, including :

In the left column of the product page (Before the "Print" button)
In the right column (After the "Add to card" block)
In the footer (after the description, before the tabs.)
As a hook, to let you place it where you want.

(note: The position will vary on the theme you use and it's configuration)
